KANSAS CITY, Mo., September 4, 2019 - Caravan Health, the national leader in population health management, today announced that Tim Gronniger has been named Chief Executive Officer and founding CEO Lynn Barr will take on the newly created role of Executive Chairwoman.

Tim Gronniger started with Caravan Health in 2017 and formerly served as the company’s President. He came to Caravan after more than a decade as a government official, most recently as Chief of Staff and Director of Delivery System Reform at the Centers for Medicare and Medicaid Services. In his new role, Mr. Gronniger will oversee all company operations and strategy.

“I joined Caravan Health two years ago because I saw the people and the company organized around helping community health systems do better for their patients,” said Tim Gronniger. “I’ve found that and more, with passionate people inside our company and among our clients working to   creatively solve problems doctors, nurses and system leaders face every day. I am thrilled to lead this team and this company into the future."

The role of Executive Chairwoman will allow Lynn Barr to focus more of her time on deep engagement in big projects, including the company’s efforts to more systematically engage patients in their care as well as her passion for improving health care in rural America. She will remain closely involved in Caravan Health corporate strategy as Chair of the company’s board.

“Caravan Health is very lucky to have an extraordinary leader like Tim Gronniger to run the company so I can spend my time working with providers and policy makers to continue to craft a better future for 20 million rural patients,” said Lynn Barr. “Rural health systems are under significant strain, with facilities closing at an alarming rate. Even though rural providers and patients benefit greatly from accountable care, if outdated rural payment policies aren’t fixed we will see more closures under value based care instead of encouraging participation.”
