Caravan Health is pleased to welcome Andy Slavitt to its third annual Accountable Care Symposium in December in Phoenix, Ariz.


Mr. Slavitt has a record of distinguished service as former Acting Administrator of the Centers for Medicare & Medicaid Services (CMS), general partner at Town Hall Ventures, and founder of United States of Care, a non-partisan non-profit dedicated to health care access. He will share his thoughts about the national health care conversation with the symposium attendees on Wednesday, December 5.

Once again, Caravan Health expects the symposium to attract hundreds of top practitioners and experts in population health and value-based payment. This one-of-a-kind event is an important opportunity to learn about how to succeed in accountable care, connect with colleagues, and hear the latest on ACO best practices and policy. The two-day event, held at the Arizona Grand Resort, will be filled with informative and dynamic speakers covering topics such as care coordination, improving physician engagement, and population health
management.

Caravan Health CEO and Founder Lynn Barr will speak alongside Milliman actuaries Steve Melek and Ally Weaver about “Managing and Winning in Risk.” Caravan Health Senior Vice President Tim Gronniger will provide an update on the latest in regulatory changes for the Medicare Shared Savings Program. Other speakers include hospital CEOs, ACO clinical care coordinators, and population health nurses and physicians engaged in direct patient care.
