This year’s symposium is packed with informative sessions about what’s happening in value-based care. 

At this eagerly anticipated annual event, healthcare leaders from across the country will gather for two days of learning, networking, and sharing real word strategies for success. Continuing education credits are now available to both nurses and physicians.  

This year’s symposium will feature educational tracks on hot topics like technology, leadership, and clinical practice transformation. 

General Session
 

Attendees will gather to hear the latest from Caravan Health leaders and invited experts. Times are changing for value-based payment. This past year has seen major policy shifts and new programs, as well as many more providers realizing time is right to jump into value-based payment. This is your chance to hear our experts tell you more about how you and your organization can maximize patient care and performance. It has also been a big year for Caravan Health, and we will catch you up on company news.

Practice Transformation

All clinical and non-clinical staff must be engaged and working at the top of their license to create outstanding quality and financial results. Every session will include stories from clients about how they overcome common challenges. This is not an academic affair.

Leadership and Strategy
 

Engaging in a new approach to care requires having the right team and the right plan in place. In a Caravan Health collaborative ACO, providers follow a proven script for success that has clear roles for executives, clinical leads, and office staff. Collaborative ACOs succeed when independent providers come together and everyone puts in the work. There is power in numbers – Caravan’s model can get you ready for risk.

Starter Sessions
 

It’s an incredibly exciting time to jump into value-based payment and accountable care. For those just getting started, the symposium offers introductory sessions for newcomers to learn from those experienced in achieving ACO success.

Sessions include: 
Population Health 101, Basics of Team-Based Care, and Introduction to HCC Coding 

 
December 11th - The Symposium begins at 8:00AM and concludes at 4:30PM

December 12th - The Symposium begins at 9:00AM with morning wellness activities beforehand. The Symposium concludes at 4:00PM.

The 2019 Accountable Care Symposium will feature more breakout sessions than ever. Further information regarding the Symposium agenda will be updated on an ongoing basis.
