The Centers for Medicare & Medicaid Services (CMS) is helping rural providers participate in accountable care at little to no cost through its Community Health Access and Rural Transformation (CHART) Model. Starting in 2022, this new ACO initiative will help rural providers in the Medicare Shared Savings Program improve quality and financial outcomes.

Caravan Health wants to help rural providers get connected to this valuable support and is now recruiting participants for a CHART ACO. Join Caravan Health’s CEO & President Tim Gronniger for a webinar to learn about the details of the program and how to apply.
