With Tim Gronniger at the helm, Caravan Health guided more than 300 health systems and 26,000 clinicians to sustainable success through innovative population health programs. Even amid the challenges during a years-long pandemic, Gronniger demonstrated that the key to a successful value-based care model is through collaboration in an accountable care organization so groups of unrelated health systems can successfully come together to reach the scale necessary to thrive using population health methodologies.
