With Tim Gronniger at the helm, Caravan Health guided more than 300 health systems and 26,000 clinicians to sustainable success through innovative population health programs. Even amid the challenges during a years-long pandemic, Gronniger demonstrated that the key to a successful value-based care model is through collaboration in an accountable care organization so groups of unrelated health systems can successfully come together to reach the scale necessary to thrive using population health methodologies.  

Over 90% of Caravan Health’s ACO clients are covered entities. As a way to help its rural providers provide a backstop against the unpredictable health care landscape, Gronniger led Caravan Health through the launch of a new 340B application to help CEs increase their 340B drug discounts as an added benefit of ACO participation. The 340B application was the newest addition to Caravan Coach, Caravan Health’s award-winning ACO management software specifically designed to drive successful clinical workflows and create meaningful improvements in cost and quality. With Gronniger’s guidance, Caravan Coach is industry-leading and its innovative 340B application has helped safety net providers stretch limited resources and succeed in value-based care.
