On May 5, 2022, Caravan Health Founder and Signify Chief Innovation Officer, Lynn Barr, presented an informational webinar about the New Realizing Access, Equity, and Community Health (REACH) ACO opportunity for 2023. The webinar addressed the question of how safety net hospitals can succeed in a REACH ACO. Last month Signify Health, with the support of Caravan, submitted its REACH ACO application to further support safety net health systems in accountable care.

What is ACO REACH?: REACH was unveiled in February 2022 as a re-design of the Global and Professional tracks of the Direct Contracting model. ACO REACH highlights the voices of beneficiaries and providers and places more focus on reducing our nation’s health disparities.
For years, Caravan Health supported ACOs in the Shared Savings Program have excelled at the population health basics, shared governance with providers and beneficiaries successfully, and have earned impressive shared savings. Earlier this year, Caravan Health joined forces with Signify Health to help providers manage total cost of care. REACH will help safety net providers succeed in accountable care. ACO REACH is designed to better serve the underserved. Each REACH ACO must have a health equity plan approved by CMS showing how providers will address hard-to-reach patient populations. Safety net providers have been performing this work all along and this program rewards those efforts. REACH includes specific benefit flexibilities such as waived part B copays, home health and telehealth waivers, and concurrent care for hospice patients, that aid and strengthen patient relationships.
ACO REACH providers receive per member capitated payments which unlocks a more intensive service model for higher needs patients. Since this model provides 100% shared savings and losses, more of the accrued savings go back to the providers serving the underserved.
