In March, the Chartis Center for Rural Health’s annual list of top 100 critical access hospitals in the nation. To determine the list, the Chartis Center used the Hospital Strength Index, which assesses performance in eight distinct areas: inpatient market share, outpatient market share, quality, outcomes, patient perspective, cost, charge, and financial efficiency. 

Included in the top 100 are 10 Caravan Health clients including:
Appleton Area Health is located in Appleton, Minnesota. Appleton Area is one of the newest members to the Caravan Health ACOs joining in January 2020. Appleton Area has maintained a focus on quality care to their patients in all settings during the public health emergency including Skilled Building while also creating successful pathways to build ACO workflows. 
Barrett Hospital Development Corp, located in Dillon, Montana distinguishes themselves with a visionary approach to all programs, understanding how small tasks and changes flow into the big picture and end game. 
Boone County Health Center operates four Rural Health Clinics in Albion, Nebraska, and has been with Caravan Health since 2019. They treat approximately 1,500 traditional Medicare patients in their patient population and scored among the highest in the region for ACO quality and HCC coding in 2020. High quality cost-effective patient care is their priority from their C-suite leadership to the front-line staff.
Burgess Health Center lives by their mission, “Get the best care in your own backyard”. Serving patients in Onawa, Iowa, they have several clinics located conveniently where patients live, providing high level care in a focused, patient-friendly environment. Burgess Health Center partnered with Caravan Health in 2018 and has grown in several aspects of preventive care services for their Medicare patients.
Central Montana Medical Facilities consistently strive to move the dial from good to great with an emphasis on CPC+ activities and keeping the patient at the center of all they do. 
Community Hospital of Anaconda in Anaconda, Montana is a consistent high-performer in demonstrating high quality and safe care for their patients. Their mission statement and core values of compassion, hospitality, and a job well done is at the heart of everything they do. 
Greater Regional Medical Center is located in Creston, Iowa. This highly engaged community of professionals joined Caravan Health in 2016 and maintain their philosophy of providing high-quality care in a cost-effective manner. Greater Regional consistently looks to the future to anticipate the needs of patients and creating partnerships to ensure they meet patient’s expectations. Greater Regional is one of the highest Caravan Health performers, ensuring Medicare patients receive personalized care to help them stay out of the hospital and safely at home. 
Memorial Aurora is not only a critical access hospital but also has a community health clinic in Aurora, Nebraska with approximately 1,200 traditional Medicare patients in their population. They joined Caravan Health in 2016 and were among the highest performers in their region for ACO quality and HCC coding in 2020.
North Valley Hospital is located in Whitefish, MT and recently joined Caravan Health in 2021.
West River Health Services is located in Hettinger, North Dakota and has consistently been recognized with the distinction of being leaders in rural health.

Caravan client, Barret Hospital, serves a rural community in and around Dillon, Montana and has been a Caravan Health client since 2016. Due to the strength in numbers, Caravan’s model has also demonstrated success with large-scale systems and facilities that serve patients in urban areas. Caravan clients consistently lead all accountable care providers in shared savings and quality scores. Caravan’s client base includes more than 22,000 clinicians who serve more than 553,000 attributed Medicare patients. In 2019-2020, Caravan’s total shared savings was $120 million.
