Boardroom to Bedside: Reduce costs and improve clinical quality with data that drives integrated system change

Caravan Health holds Quality Improvement Workshops across the nation to teach providers about the services they can implement to improve health and wellness. During the interactive workshops, participants will learn about best practices to improve the health of their community, increase practice revenue, and improve quality scores. These workshops are also a great opportunity for providers to network with their peers and build relationships.
At our Summer workshops, you will work with Caravan Health experts to review and interpret your own performance data, leaving with actionable next steps customized to your practice. Our goal is to help you reduce costs and improve care within your community, while also improving your quality scores.

Workshops will also go over how to:

  • Drive system change through team-based approach care
  • Reduce costs and improve care through increasing chronic care management and management of patients in transition
  • Bring together clinical data, financial data, and utilization metrics with Compass
  • Understand your HCC Score: What is it and why is it important?

Recommended attendees
ACO Champions, Population Health Nurses, Practice Managers, members from your organization's Quality team, IT team members and any clinical leaders that can benefit from the agenda or networking opportunities.
