Caravan Health Accountable Care Symposium
Caravan Health is excited to announce its second annual Accountable Care Symposium, the premier learning and networking event for current and future participants in Advanced Payment Models. The landmark event returns to Phoenix, Arizona on December 6 and 7 at the Hilton Squaw Peak Resort, where hundreds of healthcare professionals will come together to facilitate high-level learning and thoughtful discussion about the advancement of value-based payment systems.
Caravan Health has released preliminary 2016 clinical quality improvement data for 22 rural health care systems participating in an ACO for two years. Findings show rural hospitals and clinicians can improve patient quality of life and boost potential for increased shared savings by implementing integrated population health strategies under Caravan Health’s ACO model. Lynn Barr, MPH, Caravan Health CEO, along with Rayna Caplan, Caravan Health Director of Population Health Improvement, will present a one hour webinar about the clinical data. The webinar will feature presenters from the participating ACOs, where they will discuss strategies employed to strengthen their delivery systems.
